(3R,4S,5S,6R,7R,9R,11S,12R,13S,14R)-6-[(2S,3R,4S,6R)-4-dimethylamino-3-hydroxy-6-methyloxan-2-yl]oxy-14-ethyl-7,12,13-trihydroxy-4-[(2R,4R,5S,6S)-5-hydroxy-4-methoxy-4,6-dimethyloxan-2-yl]oxy-10-(2-methoxyethoxymethoxyimino)-3,5,7,9,11,13-hexamethyl-1-oxacyclotetradecan-2-one

Drug Uses Roxithromycin

Roxithromycin is a semi-synthetic macrolide antibiotic. It is very similar in composition, chemical structure and mechanism of action to erythromycin, azithromycin, or clarithromycin. Roxithromycin prevents bacteria from growing, by interfering with their protein synthesis. Roxithromycin binds to the subunit 50S of the bacterial ribosome, and thus inhibits the translocation of peptides. Roxithromycin has similar antimicrobial spectrum as erythromycin, but is more effective against certain gram-negative bacteria, particularly Legionella pneumophila. It can treat respiratory tract, urinary and soft tissue infections. This medicine is used to treat infections in different parts of the body caused by bacteria.
For example:
1.acute pharyngitis (sore throat and discomfort when swallowing)
2.tonsillitis
3.sinusitis
4.acute bronchitis (infection of the bronchi causing coughing)
5.pneumonia (lung infection characterised by fever, malaise, headache)
6.skin and soft tissue infections
7.non gonoccocal urethritis
8.impetigo (bacterial infection causing sores on the skin)


Absorption

Very rapidly absorbed and diffused into most tissues and phagocytes.

How Taken Roxithromycin

Do not take Rulide if you have an allergy to: roxithromycin or any other macrolide antibiotic eg., azithromycin, clarithromycin or erythromycin. Any of the ingredients listed at the end of this leaflet (see product description). If you are taking certain medicines for migraine headache called ergot alkaloids.
Before you start taking this medicine tell your doctor if: you are pregnant or intend to become pregnant; you are breast-feeding or plan to breast-feed; you have or have ever had any other health problems/medical conditions, including: kidney problems (impaired function), liver problems (hepatic cirrhosis with jaundice and /or ascites)
Tell your doctor if you are taking any other medicines, including medicines that you buy without a prescription from your pharmacy, supermarket or health food shop.


Side Effects

Most common side-effects are gastrointestinal; diarrhoea, nausea, abdominal pain and vomiting. Less common side-effects include headaches, rashes, abnormal liver function values and alteration in senses of smell and taste.
If the symptoms of your infection do not improve within a few days, or if they become worse, tell your doctor.
If you get severe diarrhoea tell your doctor, pharmacist or nurse immediately. Do this even if it occurs several weeks after Rulide has been stopped. Diarrhoea may mean that you have a serious condition affecting your bowel. You may need urgent medical care. Do not take any diarrhoea medicine without first checking with your doctor.
If you get a sore, white mouth or tongue while taking or soon after stopping Rulide, tell your doctor or pharmacist. Also tell your doctor or pharmacist if you get vaginal itching or discharge.
This may mean you have a fungal/yeast infection called thrush. Sometimes the use of Rulide allows fungi/yeast to grow and the above symptoms to occur. Rulide does not work against fungi/yeast.
Tell your doctor or pharmacist as soon as possible if you do not feel well while you are taking Rulide. Rulide helps many people with bacterial infections, but it may have unwanted side effects in some people. All medicines can have side effects. Sometimes they are serious, most of the time they are not. Medical treatment may be required for some of the side effects.

Interaction

Concomitant administration contra-indicated:
Vasoconstrictive ergot alkaloids.
Concomitant administrations not recommended:
Terfenadine and astemizole: Certain macrolides interact with terfenadine and astemizole leading to increased serum concentrations of the latter. This may result in severe ventricular arrhythmia, typically torsades de pointe. Although such a reaction has not been demonstrated with roxithromycin, concomitant administration of roxithromycin with terfenadine or astemizole is not recommended. Cisapride, pimozide: Other drugs such as cisapride or pimozide, which are metabolised by hepatic CYP3A isozymes have been associated with QT interval prolongation and/or cardiac arrythmias (typically torsades de pointe) as a result of increase in their serum level subsequent to interaction with significant inhibitors of the isozyme, including some macrolide antibacterials. Although such a risk is not verified for roxithromycin, combination of roxithromycin with such drugs is not recommended.

Missed Dose

If you miss a dose: If it is almost time for your next dose, skip the dose you missed and take your next dose when you are meant to. Otherwise, take it as soon as you remember, and then go back to taking your tablets as you would normally. If you are not sure whether to skip the dose, talk to your doctor or pharmacist. Do not double a dose to make up for the dose that you missed.
